The Role We are seeking a compassionate and proactive Engagement Partner (Registered Nurse) to join our Support at Home program. In this role, you will: Engage with local hospital networks public / private discharge planners PHNs, GPs assessment agencies to build strong links & referral pathways for new & existing clients. Creation of new business and sustain current clients by accessing additional services / programs. Engage with clients, families, and referral networks to attract, retain, and optimise home care services. Collaborate with Care Partners and clinical teams to assess client needs, promote restorative care, and ensure timely transitions between care levels. Contribute to reporting, continuous improvement, and operational performance through accurate record-keeping and stakeholder engagement. Skills And Experiences Key Requirements Current AHPRA registration as a Registered Nurse. Experience in aged care, community health, or a related sector. Knowledge of the Support at Home program and the Australian aged care system. Strong interpersonal, communication, and digital literacy skills. Desirable Qualifications in aged care, community services, or a related field. Experience with restorative or reablement programs.
