As a General Practitioner (GP) employedthrough the Additional Roles Reimbursement Scheme (ARRS) within a Primary CareNetwork (PCN), you will provide high-quality, patient-centred care whileworking as part of a multidisciplinary team. This role is ideal for a recentlyqualified GP, offering the opportunity to develop your clinical skills andconfidence in a supportive environment.

You will manage a range of acute andchronic conditions, participate in health promotion and contribute toteam-based care plans, with appropriate supervision.

The post encouragesprofessional growth through regular feedback, mentoring and opportunities forfurther training in line with early-career development.

LPC are a Tier 2 Visa Sponsor and as such, welcome appropriate applications


Main duties of the job

The GP will undertake a variety of duties governed by theGMS contract and objectives of Enhanced services including:

Inperson/telephone consultations

Visitingpatients at home/in nursing homes

Checking/signing repeat prescriptions

Queries, paperwork, correspondence & clinical admin as required

Workingclosely with PCN and practice teams

Job responsibilities

Make professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers

Screen patients for disease risk factors and early signs ofillness

Cover all clinical management of patients in surgeryas appropriate including initiating investigations, reviewing results, makingreferrals to secondary care or to other providers

Develop care and treatment plans in consultation withpatients and in line with current Practice disease management protocols as wellwider guidance such as that from NICE

Appropriate follow up of patients

Record clear and contemporaneous computerised consultationnotes

Participate in clinical audits, learning event analysesand practice meetings

Compile and issue computer-generated acute and repeatprescriptions

Prescribe generically whenever appropriate inaccordance with the Practice prescribing formulary, NICE guidance and GoodPrescribing Practice

Work with/provide support for other members of theprimary health care team

Play a lead role in the practice achieving agreed/otherkey performance indicator targets


About us

We have 22 practices across 4 PCNs Blackburn North, East, West and Darwen with a population of c180,000.

Our PCNs and practice teams have a positive and supportive working relationship, and this role is an opportunity to enhance PCN services further. The new exciting roles would be integral to the PCN for fulfilling our DES responsibilities.


Job responsibilities

Main responsibilities

In person consultations

Telephone consultations

Visiting patients at home and in nursing homes

Checking and signing repeat prescriptions

Dealing with queries, paperwork and correspondence as required

Clinical administration

Working closely with our PCN and practice teams

Wider Responsibilities

Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Screen patients for disease risk factors and early signs of illness

Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate

Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE

Ensure appropriate follow up of patients

Record clear and contemporaneous computerised consultation notes to agreed standards

Participate in clinical audits, and learning event analyses and practice meetings

Compile and issue computer-generated acute and repeat prescriptions avoiding hand-written prescriptions.

Prescribe generically whenever appropriately and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance)

Work with and provide support for other members of the primary health care team

Play a lead role in the practice achieving agreed and other key performance indicator targets

Quality

Manage personal time and workload effectively and efficiently

Meet deadlines and other performance benchmarks and reporting for the PCN

Use significant event analysis to alert staff to areas of potential risk

Education, Training and Development

To undertake all relevant training

Engage with and participate fully in all mandatory training and development

Take personal ownership of the responsibility to keep up to date with latest guidelines

Undertake additional training and development to close potential gaps in skills and knowledge

Governance and Risk

Ensure the confidentiality and security of all information that is dealt with in the course of performing duties

Ensure that all systems for Information Governance, GDPR, and confidentiality is maintained at all times

Communication

The post-holder should recognise the importance of effective communication within the team and will strive to:

Communicate effectively with other team members

Communicate effectively with patients and carers

Recognise peoples needs for alternative methods of communication and respond accordingly

In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to GP practices, patients and their carers, practice staff, GP federation staff and other healthcare workers. They may also have access to information relating to the federation as a business organisation. All such information from any source is to be regarded as strictly confidential

Information relating to patients, carers, colleagues, other healthcare workers or the business of the organisation may only be divulged to authorised persons in accordance with LPC policies and procedures, IG regulations and Data Protection Act relating to confidentiality and the protection of personal and sensitive data

Smoke Free Policy

Person Specification


Experience

* Experience of working in a GP practice using EMIS
* General understanding of the primary care contract


Other

* UK Drivers licence
* Not previously been substantively employed at a GP Practice
* Observance of strict confidentiality
* Excellent interpersonal and communication skills
* Ability to listen and empathise
* Ability to work as part of a multidisciplinary team
* Competent time management and organisational skills
* Adaptable and forward looking with problem solving and a 'solutions focused' approach
* A demonstrable commitment to professional development


Qualifications

* Qualified GP with full GMC registration licence to practice, on a performers list
* Newly Qualified General Practitioner (less than 2 years from qualification)
