We are currently recruiting for a Care Manager to work within our Stirling Hospital Discharge Team within the Clackmannanshire and Stirling Health and Social Care Partnership. We welcome applications from individuals from a Social Work, AHP and Nursing background.
This post is 28 hours per week and the post holder will be line managed by the Team Manager for Hospital Discharge. You will work within an outcomes based framework to carry out individual assessments, and to plan, manage and review care needs once discharged.
This is a really existing to be part of the Hospital Discharge Team as we strive to improve flow across our hospital sites with a focus on a home first approach.
