Advanced Practitioner Clinical Pharmacist - Maternity Cover (SWaN PCN)

* Lead and co-ordinate the professionalclinical and pharmaceutical input into the safe and cost-effective managementand development of the utilisation of medication.
* Provide professional, managerial, and clinicalleadership to a team of pharmacy staff.
* Apply highly specialist knowledge to patientsin a designated setting, to provide and develop clinical services, produceprotocols or guidelines, undertake audits/research, act as a source ofpharmaceutical expertise and promote cost effective use of medicines to otherhealthcare professionals.

Main duties of the job

* Deal with issues that need senior input when acting as the mostsenior pharmacist available on site.
* Participate in the provision and development of pharmaceuticalservices to Quay Healthcare and for other units provided with services by theQuay Healthcare under service level agreements.
* When registered as a non-medical prescriber, undertake theclinical management and drug treatment of designated patients.
* Be responsible for providing formal training / education forPharmacy staff and patients, on medication use and expenditure and other areasat the request of the education lead.
* Perform risk assessments as required by the Medical Director.
* Take a lead role with NMP, quarterly reporting and review ofmedicines management indicators and clinical audit supervision.Maintaining and updating medicines governance polices including SOPs.


About us

QuayHealthcare is a non-profit company owned by the GP practices inWarrington. The PCN is headed by ourcommitted Clinical Directors, Strategic Manager, Practice Managers and GPmembers.

QuayHealthcare supports three Primary Care Networks (PCNs), based in Warrington,working collaboratively with East Warrington PCN, Central East Warrington PCN andSWAN (South Warrington PCN). There are 13 practices across the 3 PCNs.

Our visionis to provide innovative staff and services to patients and the PCNs. We havesuccessfully employed clinical pharmacists, pharmacy technicians, first contactphysiotherapists, mental health practitioners, care coordinators, and firstcontact practitioners.


Job responsibilities

* Document general and highly specialistpharmaceutical care issues and care planning in patients case notes, verballyinforming medical/nursing staff of urgent/important actions to be taken.
* Participate in the implementation,communication and delivery of the clinical governance and medicines managementobjectives of Quay Healthcare. Take the lead in implementing such objectives inown area of responsibility (e.g. Implementing and monitoring adherence to newproduct introduction guidance, NICE guidance and safe, cost effective,appropriate prescribing initiatives). Communication / consultation with MedicinesManagement Team to ensure safe prescribing.
* Present research or audit results withinpharmacy and at appropriate multidisciplinary directorate audit meetings.
* At education sessions, individual meetingswith senior medical staff advise and influence prescribers and other healthcareprofessionals on the action, properties, and the safe, cost-effective use ofmedicines.
* Using tact and diplomacy, conveyinformation and influence prescribing when medical staff are unaware ofdeveloped policy or disagree with policies sanctioned by the MedicinesGovernance Committee.
* Provide additional specialist informationand provide support/clinical supervision to junior pharmacists with respect tothese activities in designated specialism.
* Overcome barriers to communication inorder to provide proactive and reactive pharmaceutical information and adviceto prescribers, other healthcare professionals and patients taking intoconsideration their knowledge, skills, level of understanding and disabilities.
* Use tact and diplomacy when communicatingsensitive medicines information to patients who, for example, have not yet beeninformed of their diagnosis or where the medication has been started in advanceof a confirmed diagnosis.
* Provide health promotion advice topatients or carers.
* Communicate effectively with otherPharmacy colleagues to convey information that ensures continuity ofpharmaceutical care for individual patients and the availability of adequatesupply of medication to meet patients needs.
* Communicate with prescribers and/or otherhealthcare professionals & the Medical Director to determine the course ofaction to be taken when a medication incident or near miss occurs and whennecessary, discuss the incident with a patient or their relatives in order toprovide advice, guidance, or reassurance.
* Comply with Quay Healthcares Policy ondocumenting critical incidents and near misses and assist with root causeanalysis and the implementation of changes in practice to reduce risk.
* Attend staff meetings or read the minutesof staff meetings and put into effect any actions as appropriate.
* Personal & People Development.Provide clinical supervision, training and support for clinical pharmacists andqualified clinical pharmacy technicians.
* Be responsible for clinicalpharmacy/pharmacy technicians education within primary care. Encourage andprovide support (mentorship / education & training) to enable other staffto undertake continuing professional development.
* Identify the need for and providerelevant education / support to patients and/or carers.
* Undertake continuing professionaldevelopment as per national guidelines from the General Pharmaceutical Council.Maintain and update own competencies and with the Principal Pharmacist(s),identify own training needs and courses to develop within designatedspecialism.

Quality

* Lead on the development and implementationof new Pharmacy and Practice-wide guidelines and policies and the review ofexisting ones.
* Ensure that the quality and accuracy ofall work is maintained despite frequent, unpredictable, interruptions.
* Within the immediate work area, ensure allactions and activities of oneself and other pharmacy staff members areperformed and checked in accordance with current medicines legislation and withguidelines issued by the Health and Safety Executive, Medicines Inspectorate,or other appropriate body.
* Comply and ensure that other staff memberscomply with the Medicines Act, the GPhC standards for pharmacy professionalsand other relevant legislation.

Lead PharmacistWork / Medicines Management

* Support Senior Pharmacy Management withother activities associated with CQC requirements, Governance, Risk Managementand more specifically Medicines Management
* Undertake horizon scanning for newproducts about to enter the market.
* Lead on the production of writtendocumentation to support improvements in the use of medicines within PrimaryCare. Liaise and agree content with pharmacy staff representing primary careinterests and submit documentation to Quay Healthcare.
* Lead on the implementation of local andnational guidelines as required.
* To undertake the full range of duties of aline manager for any assigned pharmacists including appraisals, developmentrevies, performance management and involvement in the recruitment process.
* Towork at an advanced level in the designated areas of practice.

Research &Audit

* Undertake audits and research work for QuayHealthcare in order to develop and improve services and to ensure compliancewith local and national guidelines.
* Present audit data or practice researchwithin Pharmacy and to the Medical Director, where appropriate.


Person Specification


Qualifications

* Masters qualification in Pharmacy 4 years, or equivalent qualification, plus 1-year pre-registration training and experience.
* Member of the General Pharmaceutical Council.
* Postgraduate Certificate or Diploma in Clinical Pharmacy or equivalent documented clinical experience.
* Independent prescribing qualification (V300 or equivalent).
* Excellent IT skills in Microsoft Office applications.
* Able to work with and influence other grades of staff and professions Ability to use own initiative.
* Excellent organisational skills.
* Ability to work as part of a team.
* Ability to prioritise workload, work accurately under pressure, and meet deadlines.
* Ability to problem solve and think laterally.
* Good all-round clinical knowledge and practical clinical skills.
* Knowledge of national guidance NICE, PanMersey.
