Main duties of the job
* Manage and promote the repeat dispensing service
* Increase safety mechanisms such as audit and PDSA cycles
* Implement drug withdrawals and alerts
* Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication reviews
* Contribute to reductions in medication related hospital admissions and readmission by supporting patients to get the best outcome from their medicines and identifying and addressing medicine related issues
* Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines
* Work within the Group to undertake medication reviews particularly in high risk groups such as frail elderly, substance misuse and patients on high risk medicines
* Improve prescribing practice through educational support for all prescribers within the practice
* Run and oversee the YMG Anticoagulation service using the surgery's protocol
* Lead on where changes in evidence require changes in prescribing across patient population e.g., where a drug is withdrawn or indications change
* Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines
* To attend and contribute to meetings as requested
* To keep up to date with changes within own area of expertise and ensure appropriate personal development
* Work with wider team to ensure practices are compliant with CQC standards where medicines are involved
* Support pharmacy technicians and prescription clerks
* Contributing to achievement of QoF and locally commissioned QI/cost effective schemes
* Reviewing systems for safer prescribing including drug monitoring
* Long term condition reviews

* Independent prescriber
