This is an excellent opportunity to work as a part of the specialist frailty Multi-disciplinary team (MDT) with an admission avoidance focus. As an MDT, we take pride in delivering a patient-centred focus through timely comprehensive assessments, interventions, and discharge planning. We are looking for individuals who share a passion for working with Older people.

This post is a clinical position, where the post holders will engage in completing assessments and treatment of patients within the acute hospital setting. Key responsibilities include confidently completing therapy assessments on an individual basis. Effective and adaptable communication skills are essential, as you will work with a diverse patient group. You must be motivated and enthusiastic about promoting rehabilitation and recovery. You need to be well-organized and confident, as you will have the responsibility to provide support and guidance to other therapy support staff.

Main duties include previous experience working with older people in an acute hospital or community setting, participation in a 7-day working rota, and achieving competency levels with support and training provided. Ideally, candidates should be drivers with a full license. The role may involve assessing patients in their homes, with support from qualified clinicians.

Additional contact information:

Base Location: Poole Hospital. The post will relocate to RBH around April. Interview date: TBC.

Our values define who we are as #TeamUHD. We are investing in services across the Trust, with many being transformed and developed following merger and the New Hospital Programme. Some services may move sites either temporarily or long-term. The service will relocate to Bournemouth site in March; after relocation, travel allowances will be adjusted accordingly.
