Job overview The Mental Health Services for Older People offers inpatient and community services to people in the Rotherham area, who present with a range of mental health difficulties. We also offer specialist services such as a Young Onset Dementia Service, Memory Service, Care Home Liaison and Hospital Liaison Service. We can now offer an exciting opportunity for an enthusiastic Band 3 Support Worker to join our innovative team at the Memory Service The post holder will work as a member of the multi-professional team providing high quality healthcare support, assisting in medication reviews and completing physical health checks with clients including ECGs, Blood Pressure readings and phlebotomy. You will be required to travel independently to client’s homes across Rotherham. The Trust offers flexible working arrangements, clinical supervision, development planning and career planning. Ø Contributes to the implementation and delivery of individualised evidence-based care. Ø To maintain accurate records electronically, record new problems as they are identified and report all actions to nurse in charge. Ø The post holder is required to take personal responsibility to incorporate the 6 C vision of care into their care delivery Ø Provide feedback of current patient / service user status to appropriate members of the-professional team. Ø Develop a rapport with patients / service users and encourage them to participate in their plan of care Ø Work in partnership with patients/carers and multi-agency teams to promote Health and independence Ø Assist the relevant professional during assessment / treatment / intervention where required Ø Support the patient / service user to make use of available services and facilities.
