Joint Emergency Team Rehabilitation/ Health Care Assistant

This post will work as a part of a multi‑disciplinary team, including health and social services colleagues, to meet the needs of clients referred to the Joint Emergency Team, ensuring the highest possible standards of practice.

This post will involve assisting the physiotherapy, occupational therapy and nursing staff in delivering rehabilitation programmes, treatment sessions, and undertaking clinical nursing activities.


Main duties of the job

Key Task and Responsibilities:

* Assist patient with activities of daily living – to work closely with the registered therapists, nurses and wider MDT.
* Support the implementation of care plans; including therapy plans.
* Engage patients in therapeutic activities, monitoring their physical and mental state and progress.
* Undertake appropriately delegated clinical tasks once deemed competent (e.g. venepuncture, urinalyses, wound care, monitor blood pressure, temperature, pulse, respiratory rate, blood sugar; administering enoxaparin may be required in the future).
* Promote independence and wellbeing for the patients to regain their previous level of independence as quickly as possible; working under the direct supervision of the Registered Therapists and Nurses.
* Must have basic computer literacy and be able to document care delivered on RiO.
* Able to work independently, with a designated heavy workload, regularly under pressure and to deadlines.
* Be able to cope with occasional, potentially stressful, upsetting, or emotional situations in an empathetic manner, such as patients suffering from illnesses or pain states that may lead to anxiety and aggressive behaviour.
* Manual handling of patient and inanimate loads.

Job responsibilities

Leadership You are accountable to the Lead Nurse. This clinical role must report to the registered therapist on duty regarding the care they have delivered and any concerns they have.

Clinical

* Organise own workload under supervision of a senior therapist and nurses, providing assessment, treatment and practice sessions to patients as required.
* Ensure treatments and enquiries are prioritised and dealt with promptly and efficiently.
* Undertake treatment of a designated workload, seeking guidance from senior staff as appropriate.
* Participate in and provide clinical treatment and rehabilitation programmes without direct supervision, following professional assessment and guidance.
* Implement and evaluate therapeutic treatment plans on a case‑by‑case basis, monitor client status, make minor modifications to treatment programmes within agreed protocols and parameters, reporting significant changes to the MDT.
* Maintain effective communication with all members of the MDT regarding patient treatment aims, progress and discharge planning, providing written referrals where appropriate.
* Provide appropriate and timely information to the MDT regarding patients’ health and rehabilitation requirements and changes in presentation progress.
* Communicate effectively with patients and carers to maximise rehabilitation potential, whilst recognising limitations within the role.
* Ensure patient and staff safety during treatment through identifying risks and minimising them appropriately.
* Undertake routine maintenance and safety checks of any therapeutic equipment/apparatus as required.
* Undertake clerical and administrative duties as required by the Physiotherapy or Occupational therapy service.
* Assist in the daily running and upkeep of the base, undertaking cleaning, preparation and checking of equipment and storage areas.
* Keep updated of changes in relevant care guidelines and protocols.
* Keep accurate records of patient treatments and statistical data as required.
* Submit timely and regular data as required via the trust computer systems.
* Be responsible for the safe use of equipment and report any necessary repairs.
* Ensure that all trust policies, procedures and regulations are implemented/adhered to as appropriate.
* Participate in team and department audit activity as required.
* Deal efficiently with enquiries and provide information to a wide range of internal and external callers, including consultants, GPs, Social Services, other health care professionals, voluntary agencies, clients, and their carers.
* Communicate with clients in situations where there may be barriers to communication (e.g., interpreters, receptive/cognitive pathologies, serious illness, or excess noise).
* Liaise closely with the Lead Practitioners and other staff regarding resources and developments which affect the whole service.
* Share and cascade information with the team, keeping them abreast of changes within the organisation and the local health economy.
* Act as a focus of communication for the service and represent and promote the service both within and outside the organisation.
* Work in a coordinated, multi‑disciplinary manner, communicating effectively with other team members and the wider clinical network involved in client care.

On Call/Unsocial Hours:

* The post holder will be required to work out of hours and weekends to cover on a rota basis as required.


Person Specification


Qualifications

* NVQ Level 3 in Health and Social Care
* Previous experience in community rapid response team


Current UK driving licence with car to use

* UK driving licence


12 months NHS experience in a Health setting or other relevant experience (e.g. social care)
