Optum is seeking a Nurse Practitioner or Physician Assistant to join our HouseCalls team in King County, WA. Optum is a clinician-led care organization, creating a seamless health journey for patients across the care continuum.
As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks, along with coordinating follow-up care to enhance health and well-being.

This role offers work-life balance with flexible scheduling over a 7-day week. No on-call, weekends, or holidays required.
Primary Responsibilities
1. Conduct in-home assessments including evaluation of medical history, review of systems, medication reconciliation, vital signs, and physical exams.
2. Perform evidence-based practice screenings, including point of care testing as appropriate.
3. Identify diagnoses for care management and active medical treatment.
4. Develop a list of current and past medical conditions based on assessments.
5. Communicate findings to inform members’ PCPs about potential care gaps.
6. Identify urgent/emergent situations and intervene appropriately.
7. Educate members on disease processes, medications, and compliance.
Required Qualifications
1. Active NP or PA license or ability to obtain by start date, with no disciplinary restrictions.
2. Active national NP or PA certification or ability to obtain certification in the state of assignment by start date.
3. Ability to perform physical requirements, including lifting 30 pounds, navigating stairs, and physical assessments.
4. Reliable transportation for home visits.
Preferred Qualifications
1. Additional licensure in other areas may be requested.
2. At least 1 year of clinical experience in relevant settings.
3. Experience in gerontology, cardiology, internal medicine, or endocrinology.
4. Home health or home visit experience.
5. Ability to travel up to 5% of the time, with additional compensation incentives.
6. Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
The salary range is $104,500 to $156,000 annually, based on full-time employment and experience. Benefits include a comprehensive package, incentives, stock purchase options, and 401k contributions. Employees must remain in the role for at least 12 months, and to receive full student loan repayment benefits, must stay in the role for 36 months.
