The Role
Our service is a busy, supportive environment made up of three integrated teams: Home First, the Integrated Discharge Hub, and the Community Bed Team. We are co‑located to provide a strong multidisciplinary approach, ensuring the best possible outcomes for the people we support.
Key Responsibilities
* Undertake person‑centred wellbeing assessments
* Work collaboratively with health professionals and partner agencies
* Support timely hospital discharges and reduce delays
* Manage a varied caseload within a 7‑day service
About You
* Strong communication and interpersonal skills
* Able to work at pace and make proportionate decisions
* Organised, resilient, and confident managing competing demands
* Experience within health, care, or customer‑focused environments is desirable
