Job Description: Care Liaison Officer

Reports to: Registered Manager / Service Manager

Location: Community-based, with office and home working as required

Contract: Full-time

Role Purpose

The Community Pathway Coordinator will support the effective coordination of community-based complex care packages. The role focuses on rostering, communication with service users, families, and professionals, and providing day-to-day support to ensure continuity of care. The postholder will contribute to multidisciplinary team (MDT) meetings and assist the Registered Manager in maintaining safe, person-centred, and high-quality services.

Key Responsibilities

Care Package Coordination

* Act as a key contact for service users, families, and professionals regarding assigned care packages.
* Ensure care pathways are documented, updated, and followed in line with care plans.
* Monitor service user outcomes and promptly escalate concerns to the Registered Manager or Clinical Lead.
* Attend MDT meetings as required, providing accurate updates and representing the service professionally.

Rostering & On-Call

* Prepare and maintain rotas for assigned care packages, ensuring safe and appropriate staffing levels.
* Cover an agreed share of the on-call rota, offering out-of-hours advice and support.
* Respond to rota gaps, sickness, or emergencies, helping to ensure continuity of care.

Workforce Support

* Assist in matching existing staff to service users based on skills and compatibility.
* Support induction and orientation of staff joining care packages.
* Provide day-to-day support to staff in delivering care, escalating any issues to the Registered Manager.

Quality & Compliance

* Maintain accurate care records and rostering information in line with company policies and regulatory standards.
* Support audits, compliance checks, and quality monitoring as directed.
* Contribute to risk assessments and care plan reviews, escalating concerns as necessary.
* Promote safeguarding, safety, and dignity at all times.

Person Specification

Essential

* Experience in community healthcare, domiciliary care, or complex care settings.
* Strong organisational skills, with proven ability to manage rotas.
* Confident communicator, able to engage with families and professionals.
* Flexible and able to participate in an on-call rota.
* IT literate (rostering systems, Microsoft Office, electronic care planning).

Desirable

* Health or social care qualification (NVQ/QCF Level 3 or above).
* Experience working with children or adults with complex health needs.
* Understanding of CQC regulations and compliance standards.
* Full driving licence and access to a car.

Job Types: Full-time, Permanent

Pay: £28,000.00 per year
