The Nursing Associate will work with the Nursing team and other members of the multiprofessional team, providing and monitoring the condition and health needs of patients within their care. They will support patients in the management of long‑term conditions (Asthma, Diabetes, COPD), cervical cytology, family planning, wound management, travel immunisations, and other general treatment room duties. The role is based in the Primary Care Sheffield network and involves a mix of clinical, administrative and partnership work.
Responsibilities
* Provide person‑centred care and promote health and independence.
* Support patients with long‑term condition management, following care plans under the supervision of a Registered Nurse.
* Perform clinical observations including blood pressure, temperature, respirations, pulse, weight, height and glucose monitoring.
* Undertake cannulation, venepuncture, ECGs and administer vaccines.
* Conduct cytology and complete related training as required.
* Record accurate nutritional and fluid intake, complete monitoring templates and share information with registered nurses.
* Undertake home visits within scope of practice and when needed for the patient population.
* Ensure the privacy, dignity and safety of patients at all times.
* Recognise safeguarding concerns for vulnerable patients and raise issues in line with policy.
* Communicate effectively with patients, carers, families and multidisciplinary team members.
* Engage in service improvement, audit activities and contribute to staff meetings.
* Apply practice policies, standards and guidance to deliver best practice care.
Qualifications & Training
* Completed Foundation Degree in Health and Care (Nursing Associate).
* Nurse and Midwifery Council (NMC) registration with a valid PIN and current revalidation.
* Experience working in a health‑care setting, preferably in primary care.
* Proficiency in phlebotomy, ECGs and biometrics.
* Completed vaccination and immunisation training; cytology training if possible.
* Commitment to continual professional development and CPD requirements.
* Strong organisational, communication and interpersonal skills.

Salary: £25,940.62 per annum pro rata (perquisite, health insurance, cycle to work, e‑Learning and CPD).
Contract: Permanent, Part‑time
