Overview
This is a fantastic opportunity to join a dynamic, consultant-led Specialist Palliative Care Team that prides itself in delivering high quality care, in the right place, every time. The post holder will join the service at a time of exciting change and development to improve the provision of Palliative and End of Life Care to the residents of Bolton.
We are seeking to appoint an enthusiastic, motivated, forward thinking Specialist Doctor to work within the Specialist Palliative Care team across the community and hospital setting – the specific setting may flex with the needs of the service. The position will be a developmental role that allows the post holder to gain the skills, knowledge and competencies required to work as a Specialty Doctor in Specialist Palliative and End of Life Care. The service is part of the Community Services Division which provides a multitude of services within Bolton. Experience of working in the community setting is not essential as the successful candidate will receive a full supportive induction when they join the Division and the team.
Responsibilities
* Contribute to the provision of Specialist Palliative and End of Life Care to the people of Bolton with advanced, life-shortening disease under the supervision of the Consultants in Palliative Medicine.
* Work with the Consultants in the hospital and community settings to provide and develop Specialist Palliative Care, ensuring continuity of care for patients.
* Work with the multi-professional team, contributing to ongoing assessment and providing and monitoring care in line with service pathways, as agreed with the supervising clinician.
* Assess and monitor the condition and health needs of patients in their care on an ongoing basis, in partnership with the multi-professional team.
* Act in accordance with the GMC Code of conduct and NHS terms and conditions of service will apply.
* Deliver clinical interventions across care settings, providing advice, education and support to patients, those important to them, and other health care professionals.
* Manage complex symptoms, support the prevention of inappropriate admissions to hospital and promote Advance Care Planning.
* Contribute to the development of services, teaching and research; active participation in audit is expected and supported by the Clinical Effectiveness Department.
