A 12 Months Secondment or Fixed Term Opportunity for a Senior Social Work Practitioner in The Mental Health Adult Social Care Team.
Part Time 35 hrs per week, Band 7 ( plus enhancement of 11.8% if you are qualified AMHP)
To be a core member of a multi-disciplinary Mental Health team and contribute to the effective provision of mental health services to working age adults in Torbay.
* If you have an AMHP qualification and are on the AMHP, this will attract and enhancement on the salary of 11.8% for which there is expectation that you will commit to the Torbay and South Devon AMHP Rota for a minimum 2 duties per month
* To lead on undertaking holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust/Devon Partnership Trust standards, policies and guidelines.

To be responsible for supporting and guiding practitioner in respect of risk assessment and risk management
Guide and support staff in respect of highly complex cases offering a shared case management approach for such complex cases.

* Ensure statutory requirements and guidance; national directives and TSDNFT policy and guidance including procedures and protocols for safeguarding adults and children are met and applied in all areas of team/individual activity
* Manage the flow of work to ensure timely assessment and reviews
* Ensure involvement of people, carers, and families in assessment and support planning, ensuring that the outcomes which are important to them are central to all service delivery
* Monitor risk to timely, safe, and effective service delivery, alerting more senior managers to risk and issues as these arise or increase plan and prioritise own work and caseload, and alongside Social Work Lead Support Social Work staff within the team in this task
* To contribute to the production of team plans and to advise social work lead of practice-based developments within the Team Review the quality of practice against required standards, providing feedback and supporting and monitoring improvement where required
* To contribute to the development and implementation of systems, such as Evidence-based Practice and utilising research data
* To contribute to service development; and with the Team Lead promote new ways of working particularly with regards community based social work.

* Ensure accurate and timely record keeping by all staff in line with Data Protection Act and Caldicott principles

Client/Service User Care
To ensure client's needs are assessed holistically, taking into account their needs, such as housing, employment, education and general health within the individual care plan.
To promote service users participation in the co-design of appropriate support plans based on measurable outcomes within agreed policies and budgetary provision. This will include ensuring transparency of action plans with service users in relation to any changes or withdrawal of services as necessary.
To ensure the effective participation in the assessment and review process of other relevant people as part of the service user's network and/or other individuals either within the Integrated Mental Health Team or in other agencies (liaising with Adult Social Care, IMCA services, Health services, Police, Education services and service providers)

The importance of this is recognised through the use of the half day per month study to facilitate this
Policy and Service Development
