Integrated Care Coordinator 9027| Permanent contract | £28,598 to £31,022 per annum (Scale F) | 37 hours per week | County Hall, Norwich

Norwich Locality has an exciting opportunity within the Norwich Integrated Care Coordination (ICC) team for 1 full time post on a permanent contract

Norfolk has a long-standing commitment to ICC roles, seeing them as essential to the development of integrated neighbourhood teams and to improve collaborative integrated working. The Integrated Care Coordinators are an innovative role working across Health and Social Care teams and systems supporting colleagues from across the system to proactively identify people suitable for preventative intervention and support, networking and connecting professionals such as GP’s, Social Care, Mental Health team, Community Health, Housing colleagues and more.

You will collaborate with many different professionals and services such as GP’s, Social Prescribers, Social workers, Nurses, voluntary services and housing colleagues, fostering, and promoting multi-agency working, networking, and maintaining relationships are key parts of the role. A positive can-do attitude is essential alongside professional curiosity, you will be expected to gain a good knowledge of community resources, understand the local population need and build and maintain good relationships across Norwich. You will be an excellent communicator, adaptable, flexible, and open to change which is sometimes fast paced.

You will be part of a wider team of ICCs and may be required to support other ICC services where needed, you will work closely with colleagues in the community as you support the development of Integrated Neighbourhood Teams.

As an ICC, you will plan and coordinate multi-disciplinary meetings often held in GP practice, you will monitor and track the persons journey and support colleagues to manage complex situations using local system and population health data, system and service knowledge, networking and connecting professionals together to support the individual and making referrals to appropriate support. You will work with multiple electronic systems for which training will be given. You will be able to communicate clearly and appropriately with members of the public, colleagues, and staff in other agencies – verbally, on the telephone and in writing.

You will be highly motivated and enthusiastic candidates and will have a high level of education (e.g., NVQ level 3, GCSE Levels or equivalent) and previous experience of working in voluntary sector, primary care, the NHS, or Social Care.

You will have an awareness and understanding of relevant health and social care legislation, excellent interpersonal and organisational skills along with a high level of administrative and IT skills.

The service operates a blended model of office base and virtual working, currently working from multiple sites, County Hall, City Hall, GP surgeries, local community settings, and you will need to be flexible to change. You must be able to make your own travel arrangements to a variety of destinations across the locality, not always accessible by public transport.
