This is a rewarding opportunity to play a vital role in supporting patients through their discharge journey, ensuring they receive seamless, coordinated care across health, social, and community services.</p><p>As a Care Navigator, you will act as a key link between services, working collaboratively with multidisciplinary teams to help individuals access the right support at the right time. You will build strong relationships, advocate for patients, and help remove barriers to recovery and independence.</p><p>Key responsibilities include:</p><ul><li>Build and maintain relationships with community, health, and social services.</li><li>Problem solve and locate resources to support the discharge process.</li><li>Advocate for patients and champion their needs, enabling recovery.</li><li>Act as a link between multidisciplinary teams and ensure patients experience seamless, joined-up care and support.</li></ul><p>We pride ourselves on ensuring our team’s wellbeing is put first and offering wellness opportunities including flexible working.</p><p>We are committed to the Greener NHS national ambition and to a diverse, inclusive workforce.
