An exciting opportunity has arisen for a Care Coordinator to join our team of Primary Care Network employees, helping to deliver administrative care to the patients primarily within the Hadleigh Practice (Corfe Mullen and Broadstone) but also across the Poole North Primary Care Network to patients at Birchwood Medical Practice, Harvey Practice and Canford Heath Group Practice.
The role is rewarding and varied with a combination of administrative work and patient contact, including but not limited to:
Registering carers, identifying carers and making annual wellbeing call/text and visiting Carer's Group when needed. Keeping the Carer's Board up to date. Getting involved with Carers Rights day, Young Carers Action Day, and Carers Week, dementia roadshows etc.
Patient Participation Group - Organising agenda, minutes and any admin associated.
Helping at the Leg Club as the requested. helping volunteers and supporting patients.
Getting involved in projects such as Care Navigation.
Helping and supporting Covid Clinics
Helping with the content of Practice News letter
Updating waiting room media screen.
Sending out cancer information letters
Performing dementia and frailty reviews
Summarising records
Helping with safeguarding administration
Ensuring veterans' information is up to date.
Production of posters about practice and network services.
Attention to detail to provide administrative support and respond to a patient's needs is essential although full training will be given.
