Band 7 Community Learning Disability Nurse
The Intensive Support Team (IST) is a service designed to support individuals with a learning disability who are at risk of a placement breakdown or admission to hospital. The team meets the holistic needs of service users, families and carers, aiming to avoid placement breakdown and hospital admission where possible. Where admission is unavoidable, IST offers support into inpatient settings.
We are looking for a senior nurse with the passion, person‑centred and clinical skills to join our innovative IST. IST is a multi‑disciplinary team led by a consultant nurse, matron and psychologist, and works in conjunction with community learning disability teams.
Responsibilities
* Provide day‑to‑day clinical and operational leadership to an all‑age intensive support team.
* Deliver the enhanced model of support daily throughout the year, serving the most vulnerable people in the community at risk of placement breakdown or admission to a mental health hospital.
* Work as part of a leadership multi‑disciplinary group of senior nurses and allied health professionals to reduce avoidable admissions to hospital and breakdown of community care, and provide support for those in hospital.
* Utilise sound clinical and leadership skills to ensure an evidence‑based approach to delivering specialised clinical interventions, including positive behavioural support, specialised adapted mental health assessments and interventions, and robust risk management approaches.
