Responsibilities include, but are not limited to:
* Working as part of an integrated and supportive team, planning, monitoring and evaluating work with the Multi-Disciplinary Team (MDT).
* Adhering to Social Work England standards of registration, professional values, and code of conduct to provide social work input and support to adults, carers, and families, promoting self-determination, choice, and autonomy.
* Carrying out needs-led assessments, complex work, care and support planning and reviews, and initiating multidisciplinary assessments where appropriate.
* Providing social work support to adults and their carers and families, involving them in planning and actions, and signposting or referring to other services when appropriate to develop social resilience.
* Communicating complex assessment information, including care and support plans and risk factors, to stakeholders using various methods, including ICT, in line with Information Governance.
* Communicating sensitive issues skillfully and addressing barriers to participation and understanding.
* Participating in the development and delivery of educational materials for staff and partners when appropriate.
* Ensuring applications for appointeeships and advocacy are made in accordance with needs, rights, and entitlements.
Requirements:
* Social Work Degree
* Registration with Social Work England
* Completion of Assessed and Supported Year in Employment plus 2 years’ post-qualification experience
* Ability to demonstrate practice supporting core social work values and principles
* Experience in safeguarding enquiries
* Strong verbal and written communication skills with diverse audiences including families and professionals
* Awareness of current issues and challenges in social work practice
* Ability to work five out of seven days on a rota, including weekends and unsocial hours
* Enhanced DBS check within the last 12 months or willingness to obtain one; fast track options available if on the update service
