Withan ethos of teamwork and collaboration, Lower Valley PCN provides resources andservices to support our member Practices deliver better care for patients.

Our goal is to provide excellentservices and outcomes for the patients and communities by working more closelyacross local health, social and community care organisations.

Our Care Coordinators play acrucial part in achieving this and are integralthe PCNs multidisciplinary team, working alongside Social Prescribing LinkWorkers, Physiotherapists and our Pharmacists to provide an all-encompassingapproach to personalised care services.

Ourideal candidate will have good communication and interpersonal skills, becomputer literate and work well within a team.


Main duties of the job

Job Summary

Undertake a variety of administrativeduties to assist in the smooth running of the PCN, including provision ofclerical support and informprocessing.

Support the coordination of care in care homes,including the delivery of care home rounds.

Act as the first point of call to patients andvisitors to the Lower Valley Primary Care (PCN) building, projecting apositive, welcoming image to patients and visitors, either in person or via thetelephone.

Receive, assist and direct patients in accessingthe appropriate service or healthcare professional in a courteous, efficientand effective way.

Hours: 37.5hrs, Monday Friday, 8am 4pm

Job responsibilities

Undertake work in line with PCN directedpriorities.

Act as the first port of call forpatients in relation to their care.

Ensure regular and consistentcommunication with the referrer regarding patient progress and anycomplications or guidance.

Administer tasks and ensure referrals arecompleted and care delivered by keeping in regular telephone contact.

Develop and maintain engagement withpractice staff and encourage best practice.

Provide coordination and navigation for patientsacross health and care services, alongside working closely with other primarycare roles.

Support the coordination and delivery ofcare home MDTs within the PCN, supporting delivery of the enhanced health incare homes model.

Support the PCN to collate patient andcarer feedback on their experiences.

Support national screening programmes.

Support PCN and patient liaison with otheragencies to coordinate care for patients.

Where required, proactively identify andwork with a cohort of people to support their personalised care requirements,using the available decision support aids.

Refer to patients to PCN socialprescribing link workers where a patient is identified as potentiallybenefting from this service.

Help people to manage their needs,answering their queries and support them to make appointments.

Support the delivery of personalisedcare in care homes/LD homes through collaborative working between health,social care, voluntary, community and social enterprise sector and care homepartners.

Ensure that people have good qualityinformation to help them make choices about their care.

Signpost people to accessself-management education courses, peer support or interventions that supportthem in their health and wellbeing.

In the course of seeking treatment,patients entrust us with, or allow us to gather, sensitive information inrelation to their health and other matters. They do so in confidence and havethe right to expect that staff will respect their privacy and actappropriately.

In the performance of the duties outlinein this job description the post-holder may have access to confidentialinformation relating to patients, their carers, practice, or staff information.All such information from any source is to be regarded as strictlyconfidential.

Information relating to patients,carers, colleagues, or member practices may only be divulged to authorisedperson in accordance with PGPA policies and procedures relating toconfidentiality and the protection of personal and sensitive data, or under theguidance of your manager.

Tomaintain up to date knowledge of legislation, national and local policies andissues in relation to the specific client group, mental health and communitycare delivery.

Tocomply with the Professional Codes of Conduct and to be aware of changes inthese. To maintain up to date knowledge of all relevant legislation and localpolicies and procedures implementing this.

To ensure that all duties arecarried out to the highest standard and in accordance with currently qualityinitiatives within the work area.

To comply with all relevant organisationspolicies, procedures and guidelines, including those relating to EqualOpportunities, Health and Safety and Confidentiality of Information and to beaware of any changes in these.

To comply at all times withthe PGPAs Information Governance related policies. Staff are required torespect the confidentiality of information about staff, patients and PGPAbusiness and in particular the confidentiality and security of personalidentifiable information in line with the Data Protection Act. All staff isresponsible for ensuring that any data created by them is timely,comprehensive, accurate, and fit for the purposes for which it is intended.

Inductiontraining

Onarrival at the practice, all personnel are to complete a practice inductionprogramme. This is managed by the Practice Manager.

Learningand development

Theeffective use of training and development is fundamental in ensuring that allstaff are equipped with the appropriate skills, knowledge, attitude andcompetences to perform their role. All staff will be required to partake andcomplete mandatory training as directed by the training coordinator, as well asparticipating in the practice training programmes. Staff will also be permitted(subject to approval) to undertake external training courses which will enhancetheir knowledge and skills, progress their career and ultimately enable them to improve processes and service delivery.

Staffwill be required to participate in regular supervision in accordance with goodpractice guidelines and policy and take part in the organisations annual appraisalprocess.

Allstaff are to recognise the significance of collaborative working. Teamwork isessential in multidisciplinary environments. Effective communication isessential, and all staff must ensure that they communicate in a manner whichenable the sharing of information in an appropriate manner.

Servicedelivery

Staffat PGPA must adhere to the information contained within the practice and PGPApolicies and regional directives, ensuring that protocols are adhered to at alltimes. Staff will be given detailed information during the induction provesregarding policy and procedure.

Security

Thesecurity of PGPA is the responsibility of all personnel. Staff must ensure thatthey remain vigilant at all times and report any suspicious activityimmediately to their line manager. Under no circumstances are staff to sharecodes for the door locks with anyone and they are to ensure that restrictedareas remain effectively secured.

Professionalconduct

AtPGPA, staff are required to dress appropriately for their role. Clinical staffmust dress in accordance with their role.

Leave

Allpersonnel are entitled to take leave. Line managers are to ensure all of theirstaff are afforded the opportunity to take a minimum of PGPA days leave eachyear and should be encouraged to take their leave entitlement.

This is not an exhaustives list of duties and responsibilities, and the post holder may berequired to undertake other duties, which fall within the grade of the job, indiscussion with the line manager.

Person Specification


Qualifications

* ECDL or equivalent Diploma/HNC level (or relevant experience)
* Demonstrable commitment to professional and personal continuous development.
* Knowledge of primary care IT Systems
* Qualified to NVQ level 2 in Health and Social Care.


Experience

* Experience of working with healthcare professional and or previous experience in the NHS or social care or relevant field (including unpaid work)
* Experience of data collection and providing monitoring information to assess the impact of services.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.
* Working in a setting where influence and negotiation is required.
* Experience of working with or in general practice.
* Knowledge/familiarity with medical terminology.
* Experience of supporting people, their families and carers in a related role (including unpaid work).
* Experience in use of databases.
* Vulnerable adults awareness.
* Experience of care of the elderly.


Other requirements

* Maintain confidentiality at all times.
* Flexibility to work outside core office hours.
* Full UK driving licence.


Skills and Knowledge

* Advanced experience of using word, excel and PowerPoint including ability to use word processing skills, emails and the internet to create simple plans and reports.
* Knowledge of general practice clinical systems, such as, EMIS and SystmOne Ability to read large amounts of information and extract the salient points.
* Knowledge of the personalised care approach.
* Creative problem solver and willing to search for hard-to-find information.
* Continued commitment to improve skills and ability in new areas of work.
* To review data and report on findings.
