About the Organisation
New Vision Bradford is a partnership between the Bridge Project, Create Strength Group, Project 6 and Waythrough. Under this umbrella, we deliver an adult drug and alcohol service for the Bradford and Craven District, aiming to help people tackle the use of alcohol and drugs and reduce risk‑taking behaviours through prevention, intervention and support.
Role Overview
This is a newly created outreach role within the Treatment Team. You will support clients who, after assessment by the service, require home interventions due to personal, social or medical circumstances. As an integral member of the multi‑disciplinary team, you will deliver high‑quality substance misuse treatment directly to service users within their homes or residential care settings, guiding them from initial entry through to completion of their treatment and recovery journey.
Responsibilities
* Act as the main care coordinator for an assigned group of service users.
* Provide screening, asset mapping and comprehensive assessments.
* Develop and review recovery plans with clients.
* Coordinate care across the multi‑disciplinary team to ensure a high‑quality service is maintained.
* Maintain accurate case records and progress notes.
* Support clients in managing their time and resources for their recovery journey.
Qualifications & Experience
* Minimum 2 years full‑time experience working with people with alcohol and/or drug problems and/or complex and multiple needs within the last 4 years.
* Demonstrable experience of case management and delivering care coordination in a drug and alcohol or related field.
* Excellent communication skills and the ability to manage time effectively.
* Highly organised and able to work cooperatively with colleagues.
* Willingness to undergo an enhanced DBS check.
* Valid driving licence, vehicle use for work purposes and business insurance.
