We are looking to recruit a qualified SCPHN School Nurse to join our friendly West Sussex Sussex School Nurse team.
The post holder will work within a skill mixed team to promote the health and wellbeing ofchildren, families, and communities. The post holder will provide specialist practitioner knowledge and advice, assessing health needs, developing, implementing and evaluating evidence-based care and public health interventions in a variety of settings, including homes, schools, workplaces and community environments to improve outcomes for children and their families. The role involves close collaboration with multi-agency partnersto address health inequalities and support safeguarding responsibilities.
Key Responsibilities
* Work with the team to plan and deliver a commissioned healthy child programme 0-19.
* Identifies health needs by undertaking an assessment in partnership with clients, to deliver an individual programme of care, delegating support as appropriate.
* Keep accurate, legible, contemporaneous records and written reports as required.
* Work in a community- based setting communicating with all members of Multidisciplinary Care Team, voluntary and statutory organisations, clients, members of the public and a range of other professional colleagues.
* Ensure effective liaison and communication with other professional groups within the Trust and outside agencies, works in partnership in the interests of clients.
* Build trusting relationships with children, carers and families, to positively influence their future health outcomes.
* Advocate for fairness, equity and social justice and challenge discriminatory practices and behaviours.
* Produce and present accurate objective reports at child protection conferences.
