Community Based | Outreach & Office Working
* Full-Time starting at 37.5 hours per week
* Salary up to £31,554 (after induction and confirmation of the ability to lone work for sleep ins)
* Bristol Area
* Shifts will start Sunday to Thursday 10am to 6pm
* Sleep in shifts will begin once training and induction are complete
* The above salary is base + sleep-in element
* Total hours will increase to 47.5 hours

We are looking for a compassionate and motivated Support Navigator to join a dedicated support team working with individuals who may be experiencing mental health challenges, vulnerability, or barriers to independence. The Role
As a Support Navigator, you will manage a caseload of individuals, providing tailored support that empowers people to build confidence, develop life skills, and access opportunities within their local community.
No two days are the same - you could be helping someone attend appointments, supporting them to manage their tenancy or finances, working through support plans, or helping them take positive steps towards education, employment, and independent living.
You will work closely with external professionals, families, and support networks to ensure individuals receive the right level of support while promoting recovery, wellbeing, and long-term independence.
Key Responsibilities
* Manage and support a caseload of service users
* Create and review person-centred support plans and risk assessments
* Promote independence, wellbeing, and positive outcomes
* Support individuals to access education, employment, and community opportunities
* Work collaboratively with healthcare professionals and external agencies
* Help manage budgets, appointments, and daily living tasks
* Respond to safeguarding concerns and support crisis management
* Maintain accurate records and ensure compliance standards are met
* Build positive relationships with service users, families, and colleagues
What We're Looking For
* Experience working within health, wellbeing, housing, or support services
* A compassionate and resilient approach
* Strong communication and relationship-building skills
* Good IT and organisational skills
* Understanding of mental health and person-centred support
* Ability to work independently and as part of a team
* NVQ in Health & Social Care desirable but not essential
Essential Requirements
* Full UK driving licence with access to a vehicle
* Enhanced DBS check
* Right to work in the UK
