Are you an experienced and compassionate practitioner with a passion for delivering high-quality, person-centred care? 3D Recruit is seeking a Senior Specialist CHC Assessor to join Medway Council’s Adult Social Care service. This hybrid role offers the opportunity to lead complex assessments, shape care planning, and make a genuine difference to people’s lives across the community.
About the Role:
As a Senior Specialist CHC Assessor, you’ll lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare (CHC) funding. You’ll manage complex cases, including disputes, joint funding, and Section 117 reviews, while supporting individuals and families with empathy and professionalism.
Working closely with internal teams, care providers, and partner agencies, you’ll ensure appropriate care packages are in place—whether or not individuals meet CHC criteria. You’ll also contribute to service improvement initiatives, performance targets, and staff development across the team.
Key Responsibilities:

Lead and coordinate CHC assessments and reviews (virtual and face-to-face)
*
Manage a clinical caseload from assessment to resolution
*
Collaborate with stakeholders to arrange care packages and placements
*
Provide expert advice on CHC, Section 117, and joint funding pathways
*
Maintain accurate records and support data reporting and governance
*
Contribute to training, induction, and continuous improvement initiatives
✅ What We’re Looking For:
*
Registration with Social Work England (SWE) or Nursing and Midwifery Council (NMC)
*
Substantial post-qualification experience in social work or nursing
*
Strong knowledge of CHC, Mental Capacity Act (MCA), and consent processes
*
Excellent communication, report writing, and IT skills
*
Ability to work independently and manage a dynamic caseload
*
Full UK driving licence and access to transport
Desirable:
*
Experience with Section 117 aftercare
*
Background in mental health, learning disabilities, or older adult services
