We are looking to appoint a highly motivated lead practice nurse to join our friendly supportive team at Whitchurch Medical Centre. The practice has a population of approximately 11,000 patients. The team consists of 5 Partners and 5 Sessional GP, 3 Practice Nurses and 3 Health Care Assistants. We have 4 days cover of cluster and practice pharmacist and a supportive team of cluster MSK practitioners.
We are proud to be a ‘research ready’, a training practice for GP trainees, medical students and nursing students. The candidate must be fully registered with the NMC and must have a Registered General Nurse qualification.

The successful candidate’s role will involve undertaking a range of practice nurse duties.
Job Summary
The Lead Practice Nurse will provide clinical leadership and management to the nursing team, ensuring the delivery of safe, high-quality, evidence-based care to our patient population. The post holder will manage their own clinical workload, lead on chronic disease management, and take a proactive role in practice development, clinical governance, and quality improvement initiatives.
Key Responsibilities
Leadership and Management
* Provide day-to-day clinical leadership and management support to the nursing team.
* Participate in staff appraisals, supervision, mentoring, and professional development.
* Ensure the nursing team adheres to practice policies, protocols, and standards.
* Contribute to service planning and development in line with practice and local health priorities.
* Lead on infection prevention and control within the practice.
Clinical Duties
* Deliver high standards of care in chronic disease management (e.g. diabetes, asthma, COPD, hypertension).
* Undertake general treatment room duties including wound care, immunisations, cervical cytology, family planning advice, and travel health.
* Carry out health assessments and screening activities such as new patient checks, NHS Health Checks, and over-75 reviews.
* Provide support and education to patients on self-management and lifestyle advice.
* Participate in the implementation of national screening and immunisation programmes.
Training and Development
* Support the training and mentoring of nursing students and newly recruited staff.
* Engage in continuing professional development (CPD) and maintain up-to-date clinical skills and knowledge.
* Participate in regular audits and quality improvement projects.
Clinical Governance
* Ensure accurate and timely documentation of all patient encounters using EMIS.
* Work within the scope of practice and follow relevant clinical guidelines and protocols.
* Understand and implement Patient Group Directions (PGDs) and associated policies.
* Take a lead role in safeguarding procedures and multi-disciplinary team discussions.
