Shore Medical is recruiting an experienced Health Care Assistant- Frailty to be part of the Primary Health Care Team providing high quality and forward-thinking nursing care to meet the holistic healthcare needs of the Practice population, working to agreed Practice standards and protocols for the delivery of patient care.

This role is based at our Parkstone Tower Practice in Poole and is a full time permanent position.


Overview

Shore Medical is a GP Super Partnership with 6 practices across Poole and Bournemouth. We aspire to offer exceptional care to our 58,000 patients and are innovative in our approach to developing new teams and pathways to ever improve how general practice is delivered.


Responsibilities

* Works under indirect supervision of the Practice Nurses and Nursing Lead
* Is accountable to the Nursing Lead and the Primary Care Network for their actions in undertaking delegated work.
* You will be responsible for attending to patients who wish to be seen by a health care professional within the Patients Home, providing high quality evidence-based nursing services to the population of Shore Medical.
* Provides support to other members of the primary health care team and contributes towards the co-ordination of team activities and the development of the Practice as a whole.
* Refers to more senior members of the Primary Health Care team as appropriate.


Clinical Responsibilities

The Health Care Assistant undertakes procedures that are agreed by the Head of Nursing which are likely to include:

* Venepuncture
* BP checks, height, weight, BMI, urinalysis, ECG
* Long term condition/Chronic disease health checks
* Flu & Covid vaccination (following approved training and competence checks)
* Diabetic Foot Checks (following approved training and competence checks)
* Dressings (following approved training and competence checks)

Qualifications

* Previous experience of working within a Primary Care Setting


Experience

* Evidence of effective team working
* Proficient IT and presentation skills
* Ability to write and maintain accurate records
* Team player
* Able to perform under pressure
* Professionalism and advocate for high standards
* Flexible and positive approach to work
* Able to meet travel requirements of post
* Full UK Driving License and access to own vehicle
