Job Summary
The North Liverpool Primary Care Network would like to recruit a Wellbeing Link Worker (Social Prescriber) to form a new team. This role will be based across the seventeen member practices of the PCN.
Responsibilities
Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who provide time, focus on what matters to them, and take a holistic approach to individuals’ health and wellbeing. The post holder will:
* Refer patients to community groups and statutory services for practical and emotional support.
* Support existing groups to be accessible and sustainable, and help people start new community groups in collaboration with local partners.
* Serve as a key part of the PCN multi‑disciplinary team, strengthening community and personal resilience.
* Address wider determinants of health such as debt, housing, and physical inactivity to reduce inequalities.
* Provide information and support, and act as the link between the patient, GP and other service providers.
* Manage and prioritise own caseload in accordance with needs, priorities and urgent support requirements.
* Promote lifestyle changes and prevent ill‑health through behavioural change techniques.
Essential Qualifications
* Educated to GCSE level or equivalent.
* Experience of working with the general public.
* Experience of working in a health and social care setting or with relevant voluntary sector organisations.
* Experience of delivering lifestyle changes interventions.
* Experience of working with vulnerable people.
* Excellent communication skills (written and oral).
* Strong IT skills, competent in the use of Office and Outlook.
* Effective time management (planning and organising).
* Ability to work as a team member and autonomously.
* Excellent interpersonal and listening skills.
* Problem‑solving and analytical skills.
* Ability to follow policy and procedure.
* General understanding of local well‑being/community services.
* Polite and confident.
* Flexible and cooperative.
* Forward thinker.
* High levels of integrity and professionalism.
* Sensitive and empathetic in distressing situations.
* Ability to work under pressure.
* Flexibility to work outside core office hours.

Desirable Qualifications
* Motivational interviewing/ coaching qualification.
* NVQ Level 2 in Health and Social Care or other relevant qualification.
* Experience of working as a health trainer or related link worker role.
* Ability to use EMIS patient systems.
* Good understanding of interventions, behavioural and motivational change methodologies.
* Good understanding of holistic approach to social prescribing and wider determinants of health.
Additional Requirements
* Disclosure and Barring Service Check subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
* Current UK professional registration.
Working Pattern & Compensation
* Contract: Permanent.
* Working pattern: Full‑time.
* Salary: Depending on experience.
