Fixed Term, Full Time

£46,574 up to £56,027 per annum

The Team

The Multi-Disciplinary Team (MDT) comprises of subject matter experts specialising in housing, domestic abuse, education, drug and alcohol support, youth work and mental health for adults and children.

MDT workers bring specialist knowledge and therapeutic insight. Their expertise is instrumental in re-engaging families, restoring hope, and fostering sustainable change. This team’s work is not only impactful — it’s transformative.

We are seeking an experienced, motivated and passionate individual to join our team. Being led by the Team Manager, you will be working to create the conditions for sustainable change with families.

The MDT team is situated within frontline social work teams where the referrals come from. The interventions from MDT are delivered within the community or in the families’ homes.

Role Description

The Adult Mental Health practitioner will offer intensive support to individuals aged 18+ who are experiencing significant challenges such as trauma, drug/alcohol use and trauma. The hoped for outcome from the MDT support is to improve mental health/wellbeing and parenting capacity.

The Adult Mental Health practitioner will play a critical role in building effective relationships with parents/ adults and create foundations for therapeutic support and sustainable change.

The practitioner will utilise screening tools as part of the assessment stage and use this knowledge to decide interventions as well as enable access to community mental health resources.

The post holder will have an accredited professional qualification and evidence of excellent practice, participation in casework, recording and assessment.

Strong partnership working is essential for this role as it will operate in a multi-agency environment.

Accountabilities

1. Provide bespoke assessment, interventions and a range of therapeutic options to adults who are experiencing mental health difficulties.
2. Conduct holistic assessments including safety plans taking into consideration of the social, psychological, and physical factors.
3. Provide emotional and practical support to individuals in crisis or with ongoing mental health challenges.
4. Deliver a range of interventions in relation to problem-solving, behavioural activation, and relapse prevention.
5. Engage in supervision, reflective practice, and ongoing training and contribute to service improvement.
6. Work in partnership with parents / adults to support and promote their future engagement with community services.
7. Raise knowledge and awareness of adult mental health issues and preventative strategies across Children &Families(C&F) colleagues.
8. Work collaboratively with C&F colleagues to offer advice, support and consultation, in their work with families within the quadrant.
9. Deliver advice, support and consultation in respect of mental health issues, perspectives and frameworks, to other professionals.
10. Responsible for maintaining up to date records of activity on case files, produce reports, attend court and give verbal presentations as required. Where applicable, actively participate in relevant meeting regarding the family.
11. Build strong working relationships across the Council, members of other statutory and organisations and voluntary agencies.
12. Specific individual and shared targets and objectives are defined annually within the performance management framework.

The Experience You Will Bring

13. A professional qualification or equivalent accreditation in mental health or associated discipline.
14. Significant experience of using a range of therapeutic interventions with adults and families.
15. Evidence of direct engagement with adults, with successful implementation of one-to-one interventions.
16. Deep knowledge and understanding of vulnerability and safeguarding protocols.
17. Evidence of being a catalyst for change in adults and families’ lives.
18. Strong evidence of building strong partnerships, which are collaborative and influential in their nature and enable delivery.
19. Evidence of continuing professional development and expert knowledge in relevant professional area.

The role requires you to be mobile throughout a wider operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or the ability to meet the mobility requirements of the role through other means is essential.
