Clinical Triage and Assessment Conduct same-day telephone, online, and face-to-face triage consultations to assess, diagnose and manage patient presentations in line with national and local protocols. Review all incoming patient requests (AccuRx, eConsult, NHS App, and telephone) and prioritise based on clinical urgency, safety, and suitability for self-care, allied health, or GP review. Provide autonomous assessment and management for a range of acute and minor illnesses within scope of practice. Prescribe and review medication safely where qualified as an Independent Prescriber (V300). Identify red flags, safeguarding issues, and deteriorating conditions promptly and escalate appropriately. Signposting and Demand Management Deliver safe and effective clinical signposting, ensuring patients are directed to the most appropriate clinician or service (e.g., Pharmacist, Physiotherapist, Social Prescriber, Mental Health Practitioner). Support the triage managers and admin team to maintain effective patient flow, optimise access and manage daily capacity. Communicate clearly and compassionately with patients to explain triage outcomes, ensuring understanding and reassurance. Contribute to reducing unnecessary GP appointments through effective triage resolution and patient education. Proactive and Data-Driven Care Identify vulnerable, high-risk, or frequently attending patients during triage and flag for proactive or multidisciplinary review. Use clinical data and population health tools to support proactive care planning and risk stratification. Participate in quality improvement projects to enhance patient access, safety, and satisfaction. Documentation and Governance Record all encounters, triage decisions, and safety-netting advice accurately on EMIS Web. Maintain high standards of clinical documentation and follow practice coding protocols. Participate in audits, significant event reviews, and learning processes to strengthen quality and governance. Uphold confidentiality and information governance standards in line with GDPR and NHS policies. Teamwork and Professional Development Work collaboratively with GPs, nurses, and the wider PCN multidisciplinary team. Support and mentor junior staff or trainees within the triage or clinical team. Engage in clinical supervision, reflective practice, and peer review to maintain competence and learning. Participate in regular clinical and operational meetings to improve service delivery.
