Reablement Officer (MI, Reablement and Sensory Needs) - 02781

Are you passionate about supporting individuals to regain their independence and improve their quality of life? West Berkshire Council is seeking a dedicated and motivated Reablement Officer to join our Adult Social Care Team. This is an excellent opportunity to work within a supportive and forward‑thinking organisation, making a real difference to the lives of our local community.


As a Reablement Officer, you will be responsible for:

* Working collaboratively with individuals to assess their needs and develop personalised reablement plans.
* Providing practical support and guidance to enable individuals to achieve their goals and regain independence in daily living activities.
* Monitoring progress and reviewing reablement plans, making adjustments as necessary to ensure optimal outcomes.
* Collaborating with other professionals, such as occupational therapists and healthcare providers, to ensure holistic support for individuals.
* Delivering high‑quality care and support, adhering to relevant legislation and best practice standards.
* Supporting reablement practitioners to develop and assess their competency.


Qualifications

* Experience working in a similar role within a social care environment.
* Sound knowledge of reablement principles and practices.
* Excellent communication and interpersonal skills, with the ability to build positive relationships with individuals and colleagues.
* Strong assessment and care planning skills, with the ability to identify and address individual needs.
* Ability to work independently, as well as part of a multidisciplinary team.
* Ability to problem‑solve and think of solutions to barriers to independence.
* Knowledge of relevant legislation and regulatory requirements.
* Positive, can‑do attitude with a passion for empowering individuals to achieve their full potential.

This role requires all post holders to have or be willing to complete a level 3 Diploma in health and social care.

Additionally, the role covers the entirety of West Berkshire and successful applicants would need to be prepared to travel for the beginning and end of their shift to any location within West Berkshire.


Working patterns

We are recruiting for two roles (AM and PM). The working patterns for both roles are detailed below:
AM: Week 1: Monday 7am–2pm, Tuesday 7am–2pm, Wednesday Off, Thursday Off, Friday 7am–2pm, Saturday 7am–2pm, Sunday 7am–2pm. Week 2: Monday Off, Tuesday Off, Wednesday 7am–2pm, Thursday 7am–2pm, Friday Off, Saturday Off, Sunday Off.
PM: Week 1: Monday 3pm–10pm, Tuesday 3pm–10pm, Wednesday Off, Thursday Off, Friday 3pm–10pm, Saturday 3pm–10pm, Sunday 3pm–10pm. Week 2: Monday Off, Tuesday Off, Wednesday 3pm–10pm, Thursday 3pm–10pm, Friday Off, Saturday Off, Sunday Off.
