The Therapy Team within Medicine for Older People plays a crucial role in delivering holistic, responsive care to older adults across acute and frailty pathways.
Job Overview:
We are currently seeking a Band 6 Occupational Therapist to join the Medicine for Older People Therapy Team. This role is suitable for a dynamic and skilled clinician with experience in acute care or frailty pathways and an interest in community-based response services.
Key Responsibilities:
* Complete functional assessments and plan tailored interventions for older adults
* Work within acute inpatient and community frailty response teams
* Contribute to rapid response assessments aimed at preventing admissions
* Liaise with the multidisciplinary team to ensure integrated care delivery
* Support discharge planning and continuity of care in the community
* Maintain high standards of clinical documentation
Key Requirements:
* HCPC registration as an Occupational Therapist
* Minimum 6 months NHS experience, preferably in acute/frailty/community settings
* Excellent clinical reasoning and assessment skills
* Up-to-date Moving and Handling training
* Right to work in the UK
