Job Responsibilities
* Build a natural and trusted rapport with patients and families.
* Provide compassionate care and support through home visits and telephone advice.
* Support people with sensitive topics including loss, grief and dying.
Qualifications
* Excellent communication skills and sound judgement.
* Commitment to patient‑centred interdisciplinary team working.
* Palliative care experience desirable but not essential.
* DBS check and valid UK driving licence required.
