What you will do: Conduct comprehensive physiotherapy assessments within community settings, including care homes and patients' residences. Identify patients' functional needs and establish achievable rehabilitation goals under the Discharge to Assess pathway. Develop, implement, and review personalised rehabilitation and treatment plans. Work collaboratively with multidisciplinary teams to ensure coordinated and effective patient care. Monitor patient progress and adapt treatment interventions to maximise rehabilitation outcomes. Provide support, education, and guidance to patients and their families throughout the rehabilitation process. Maintain accurate and timely clinical records in accordance with professional standards. Requirements: HCPC registered Physiotherapist. Previous community rehabilitation experience is desirable. Ability to work independently within community settings. Full UK driving licence required due to travel within the community.
