Job Description
We are seeking a dedicated and compassionate Care Coordinator to join our team in Hayling Island, United Kingdom. As a Care Coordinator, you will play a vital role in ensuring our clients receive the highest quality care and support.
1. Organise and manage care schedules for clients and care professionals
2. Liaise with clients, families, and care staff to coordinate care plans and services
3. Monitor and adjust care plans as needed to ensure optimal client outcomes
4. Maintain accurate and up-to-date client records and care documentation
5. Collaborate with the Care Manager and other team members to address client needs and concerns
6. Respond promptly to changes in schedules and communicate effectively with all relevant parties
7. Handle administrative tasks, including data entry and report generation
8. Answer telephone enquiries in a professional and friendly manner
9. Be part of the On Call rota as required
10. Ensure compliance with healthcare regulations and company policies
11. Participate in team meetings and contribute to improving care delivery processes
12. Coordinate with local healthcare providers and community resources as needed
Qualifications
13. Previous experience in care coordination or scheduling, preferably in a healthcare setting
14. Excellent communication and interpersonal skills, with the ability to interact compassionately with clients and colleagues
15. Proficiency in scheduling software and Microsoft Office programmes
16. Strong organisational and time management skills, with the ability to multitask and prioritise effectively
17. Minimum of 5 GCSEs, including Maths and English, or equivalent qualifications
18. Demonstrated ability to think flexibly and use initiative in problem-solving
19. Keen attention to detail and accuracy in record-keeping
20. Knowledge of healthcare regulations and standards
21. Ability to work under pressure and handle challenging situations with empathy and professionalism
22. Valid driving licence and access to a vehicle, as local travel may be required
23. Experience in office administration is desirable
24. Willingness to undergo a DBS check and any required training
