Are you looking for an exciting change and for a secondment to a new service? Hounslow Community Healthcare has an exciting new role to support the ongoing development of the Integrated Community Response Service including the Hospital at Home, complex discharge and Urgent Community Response pathways. We are looking for an enthusiastic and motivated candidate.
The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. We have short term funding until March 2026 to continue to run a Hospital at Home pathway. The postholder will work jointly with the Discharge Hub, Integrated Community Response Service as well as the London Borough of Hounslow to ensure patients are effectively supported through the Hospital at Home pathway.
Main duties of the job
The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.
