Overview

Join to apply for the Senior Health Care Assistant role at MARY ANN EVANS HOSPICE.

The Senior Health Care Assistant plays a key role in our Community Services team. This service includes the Hospice at Home team, the Wellbeing Programme, and the Rapid Response service. The role will primarily focus on supporting patients within the Hospice at Home and Wellbeing Programme, working across a 7-day rota between 8:00am and 8:00pm. On occasion, the postholder will work within the Rapid Response team, which is a 24-hour service and may include internal night shifts from 8:00pm to 8:00am.


Responsibilities

* Provide high-quality palliative and end-of-life care to patients and their families in patients\' homes when working with the Hospice at Home team.
* Support patients with personal care, emotional wellbeing, and practical needs; deliver short periods of respite care for family members and carers.
* Collaborate with colleagues from the NHS, social services, local care homes, and voluntary sector organisations to support patients\' preferences, particularly the wish to remain and die at home.
* Work as part of a multidisciplinary team within the Wellbeing Programme to enhance quality of life, promote a reablement approach, and support independence, dignity, and comfort.
* When required, support the Rapid Response team to deliver urgent care with a qualified palliative care nurse for distressing or uncontrolled symptoms.
* Work as part of a round-the-clock care provision to ensure timely and appropriate support during crisis situations.


Qualifications and Skills

* Recognised Health Care Assistant qualification (or equivalent experience).
* Experience in delivering palliative, end-of-life, or community-based care is desirable.
* Excellent communication and interpersonal skills; ability to provide emotional support to patients and families.
* Ability to work independently and as part of a team; flexible approach to working patterns, including occasional night shifts.
* Resilient, empathetic, with strong time management and professionalism in maintaining patient confidentiality.
