Main Activities & Responsibilities:
1. Work with adults using a strength-based approach, supporting those with complex or fluctuating needs requiring intensive support to manage risks.
2. Collaborate with other professionals to ensure a multi-agency approach that promotes independence for adults and/or their carers.
3. Meet statutory requirements, adhering to best practices in line with the Care Act 2014, the Mental Capacity Act 2005, and the Mental Health Act 1983.
4. As an experienced social worker, apply relevant legislation, policies, procedures, and social work theories autonomously.
5. Support the professional development of social care workers, including newly qualified social workers and level 2 social workers, through reflective practice, mentoring, and peer support.
6. Identify and support adults and carers at risk of abuse or neglect, utilizing Making Safeguarding Personal and completing section 42 enquiries as directed.
7. Represent the Authority in court when required, including giving evidence as a witness.
8. Represent the service and advocate for adults and carers at meetings, exchanging information, making decisions, and reviewing progress.
9. Conduct meaningful conversations to determine eligibility under the Care Act 2014.
10. Engage with adults and their carers using a strengths-based approach to identify and utilize available resources to meet their needs.
