The West Dumbarton Older Adult CMHT at Goldenhill Resource Centre, Clydebank, seeks a Band 6 Care Home Liaison Nurse.
The successful candidate will provide mental health nursing advice, support, and education to care homes within West Dunbartonshire HSCP. They will collaborate with multidisciplinary colleagues in the Older Adults’ service, assess mental health needs of clients referred from care homes, liaise with community services, and coordinate care with GPs. Responsibilities include assessment, diagnostic services, ongoing care management, care plan reviews, and facilitating hospital discharges. The role also involves managing a caseload and ensuring effective communication between care homes and community services.
