We are seeking a compassionate and dedicated Social Worker to join our team. Responsibilities
* Assess clients\' needs through interviews and observations.
* Develop individualized care plans based on client assessments.
* Provide counseling and support to clients and their families.
* Facilitate access to community resources and services.
* Advocate for clients within the healthcare system and community settings.
* Monitor client progress and adjust treatment plans as necessary.
* Collaborate with multidisciplinary teams to ensure comprehensive care.
* Document all client interactions and maintain accurate case records.
Requirements
* Master\'s degree in Social Work (MSW) from an accredited institution.
* Current state licensure as a Social Worker.
* Strong knowledge of social work principles and practices.
* Excellent communication and interpersonal skills.
* Experience in case management and providing direct services to clients.
* Ability to work effectively with diverse populations.
* Strong organizational skills and attention to detail.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £18.26
Location: Cardiff, Cardiff County
