Job Description
The post holder will assist and support other members of the team in giving care and undertaking phlebotomy to patients within a designated area, under the supervision of a qualified nurse.
The role requires you to visit patients in their own home, so you must be able to meet the travel requirements. This opportunity allows you to develop additional skills within the HCSW job description and support career progression through access to formal training and clinical competence.
Responsibilities
* Promote a 'can do' attitude and determination to develop and sustain a dynamic and responsive clinical and administrative support service within a Community Hospital / Community Team setting which delivers best practice.
* Deal appropriately with interruptions, unpredictable events and behaviours.
* Organise and deliver skilled clinical support work / phlebotomy without direct supervision.
* Gain valid informed consent and work within legal frameworks with patients.
* Liaise with nursing and therapists regarding patient care, highlighting any variations to expected outcomes from agreed treatment programmes.
* Provide basic verbal and written clinical reports to the supervising Nurse and / or therapist and document patient interventions/findings in patient records in line with Trust Record Keeping Policy.
