Social Worker (9348) at Norfolk County Council
Permanent contract, 37 hours per week, salary £37,280 – £41,771 per annum (Grade I/J). Location: Attleborough, Norfolk.
We offer a £2000 welcome payment for qualified and experienced Social Workers (grade J and above) and Practice Consultants and Team Managers new to Norfolk County Council.
For experienced Social Workers new to the Council we will try to match or exceed your existing salary within the advertised grade.
The role is part of our Follow On team, working closely with patients discharged from hospital by the Home First Hub who require long‑term care planning. The team embraces our Living Well agenda, supporting people to be independent, resilient and well. We work in a multidisciplinary environment alongside health colleagues.
Responsibilities
* Assess and provide support to adults living in the community using the Care Act 2014.
* Apply the Mental Capacity Act 2005 and Safeguarding policies in practice.
* Develop and manage long‑term care plans in partnership with service users and families.
* Maintain excellent time‑management, IT skills and up‑to‑date knowledge of Council processes and procedures.
* Collaborate with Social Workers, Assistant Practitioners and Occupational Therapists across South Norfolk.
Qualifications
You must be a registered Social Worker with Social Work England and committed to a person‑centred, strengths‑based way of working. Prior experience with complex cases and familiarity with the Care Act, Mental Capacity Act and Safeguarding is desirable.
