Support Worker Care Connection are recruiting, experienced Support Workers, to support our clients in and around Bradford on Avon. As an agency Support Worker, you will have the opportunity to make a real difference in people's lives. You will be able to work across a variety of care settings in the Wiltshire area. Supporting each service user in a person-centred way always following support plans, to enable you to give the very best person centred support and help each individual to lead the most fulfilling lives. You will encourage each individual to be as independent as possible and to develop new skills This is a temporary, ad hoc role, you will be supporting our clients with staffing gaps due to sickness and holidays. You will follow support plans to ensure person centred support, and encourage independence and choice in all aspects of daily life. Shifts here are between 07.00-22.00, they also have a sleep in shift at this service. Job Role: A Support Worker is required to deliver support within an adult's health or social care environment, supporting with daily living tasks or development of new skills. The tasks may be one to one with an adult or with other support workers or health professionals, they may be provided within the home and garden itself or outside in the community, equipment may be required for moving and handling or health tasks. A support plan on site must be followed for completion of tasks, safety, client communication and any end-of-life instruction where relevant.
