There is an opportunity for a qualified Mental Health Nurse/Social Worker/Occupational Therapist to join the recently developed In-reach/Outreach service at Carlton Court Hospital. We are seeking motivated and enthusiastic individuals who have a passion for supporting people with dementia who are living in care homes and undergoing a transition from in-patient wards to care home settings. The role will also involve supporting and educating staff in the homes, case management and leading formulation meetings. You will work alongside registered and unregistered staff, plus a clinical psychologist.
The focus of the team’s approach will mean using a psychological model which will encompass utilising skills of observation, communication and the ability to collaborate with others. We aim to develop strategies in order to support people with dementia who are experiencing distressed behaviours and behaviours that the care home staff find challenging.
Experience of working with the person with dementia is essential. There will however be the opportunity for self-development within a very supportive and positive team. We cover the Lowestoft, Waveney and Great Yarmouth area.
Responsibilities
* Role of care coordinator
* Working with in-patient wards and supporting the transition from hospital to community
* Attending review and section 117 meetings
* Observation and information gathering about the service user
* Utilising ‘The Newcastle Model’ Framework
* Coaching and education of staff within care homes and wards
* Care planning and developing behaviour support plans
* Building rapports and positive working relationships with care home staff
* Supervision of team members
* Attending team meetings and participating in reflective discussions
* Gathering life story and utilising this within a plan of care
