Overview

Applications are invited for the post of Specialty Doctor in Eldercare at the Royal Cornwall Hospitals Trust. Applications are invited from enthusiastic and dynamic individuals who are interested in assisting with the expansion of the Older Person’s Service from the Royal Cornwall Hospital in all localities in Cornwall. The Cornwall Frailty Pathway and Stroke Services are in the process of rapid change following significant investment. New opportunities to work sessions across the system from acute hospitals to community hospitals are available in any area of interest. The posts are already supported by a large, friendly, progressive Older Person’s Service who is forward thinking, passionate and caring. The consultant team work alongside our exceptional team of nurses, allied health professionals, community partners and old age psychiatrists to deliver the very best multi-disciplinary care to our community. The department is dedicated to providing a first class service where the patient and relatives come first. The Eldercare team is dedicated to providing a first-class service where the patient and relatives come first. The Eldercare team take a proactive approach to teaching and innovation as well as collaboration with partners in the community and primary care — your chance to take a lead in forging pioneering new approaches in care of older people.


Responsibilities

* The post holder will provide clinical cover for the in-patient wards and also work as an integral part of a Front Door Frailty Team in the Medical Rapid Assessment and Triage (MRATs) and Emergency Department.
* Support advanced care planning and care pathways for the patients, carers and their families.
* Work closely with both acute geriatrician teams and the primary and community locality teams.
* Supervision of junior medical staff; provide leadership to the multi-disciplinary team; contribute to teaching or trainer activities where possible.


Qualifications and requirements

* Strong communication skills and the ability to work with acute geriatrician teams and primary/community locality teams.
* Aptitude for teaching or prior trainer status would be highly desirable.
* Identity checks, professional registration and qualification checks, employment history and reference checks, work health assessments, criminal record check standards, and right to work checks are required as part of NHS employment checks.
* Experience with care planning and collaboration across multidisciplinary teams is preferred.
