As a Team Facilitator for Individuals with Complex Needs, you will lead and inspire a dedicated team of professionals to provide high-quality, compassionate care to individuals facing complex physical, emotional, and cognitive challenges. This role involves fostering an environment where both the team and the individuals we support can thrive.
You will be central to our person-centred approach, ensuring each person’s service design is tailored to their needs and delivered with empathy, dignity, and respect. Your leadership will help the team create meaningful outcomes, supporting individuals to achieve personal goals, live independently, and enjoy a fulfilling life.
This hands-on role involves overseeing daily operations, championing best practices, maintaining high standards of care, and supporting your team’s professional development. Effective communication, creative problem-solving, and building strong relationships will be essential to your success in this role.
Ultimately, as a Team Facilitator, you will shape the culture and future of care for individuals with complex needs, fostering a supportive and empowering environment where everyone feels valued and heard. You will lead by example, motivating your team to deliver excellent care and fostering continuous learning and growth.
Successful applicants will demonstrate:
* Experience working with adults or children with learning disabilities, epilepsy, autism, and physical support needs.
* Experience in using person-centred planning techniques and delivering excellent support practices.
* Strong facilitation skills to encourage staff ownership and responsibility for support quality.
* Effective communication with individuals supported, staff, families, and external professionals.
* Ability to create and deliver service designs, support strategies, risk assessments, and manage individual budgets.
* Excellent organisational skills and the ability to prioritise workload under pressure.
* SVQ Level 3 in Health and Social Care or equivalent.
* Full driving licence with access to a personal vehicle for work purposes.
