We are are looking to appoint a Salaried GP to join our friendlypractice team for 4 sessions per week. Please note carefully that the work pattern we require for this job is 2 sessions on Wednesday and 2 sessions on Friday. We also want the successful applicant to start as soon as is feasible.

Hatfield Health Centre isa long established practice. We are located in a modern, purpose built Health Centre, with a pharmacy and other community care services. We cover the local villages of Hatfield, Dunscroft,Dunsville, Hatfield Woodhouse and Lindholme and we are dedicated to providing a high quality service to our 9,500 patients.

Our team consists of 7 GPs, Physicians Assistant, Clinical Pharmacists, Pharmacist Technicians, Mental Health Nurse, Specialist Nurses, HCA, Training Nurse Associates, physiotherapists, and a range of supportive administration and reception staff.

We are rated as good across all CQC domains and a member of the East Doncaster PCN.


Main duties of the job

Undertake a variety ofduties, including surgery consultations, telephone and video consultations, home visits, care home visits, care planning and palliative care.

Electronic repeatprescriptions (EPS)

Using IT systems such as SystmOne and AccuRx for consultation/patient contact.

* Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcare workers within the organisation
* Developing care plans for health in consultation with patients andin line with current practice disease management protocols
* Admitting or discharging patients to and from the caseload andreferring to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreedstandards
* Contribute to the achievement of QOF and other locally commissioned services
* In general, the post-holder will be expected to undertake all thenormal duties and responsibilities associated with a GP working withinprimary care.

5 GP Partners, 2 Salaried GPs, Physicians Assistant, 5 Practice Nurses, 2 Clinical Pharmacists, Pharmacist Technician, Health Care Assistants, Nurse Associates, Physio, receptionists and admin support staff, Reception Supervisor, Operations Manager and Business Manager.

Job responsibilities

Clinical responsibilities:

* In accordance with the practice timetable, as agreed, thepost-holder will make themselves available to undertake a variety ofduties, including surgery consultations, telephone and video consultationsand queries, visiting patients at home, checking and authorising repeatprescriptions (EPS) and care home visits.
* Using IT systems such as SystmOne and AccuRx for consultation/patient contact.
* Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other healthcare workers within the organisation
* Assessing the health care needs of patients with undifferentiatedand undiagnosed problems
* Screening patients for disease risk factors and early signs ofillness
* Managing care home patients and producing care plans.
* Management of palliative care patients
* COVID-19 duties in line with other clinicians when necessary
* Developing care plans for health in consultation with patients andin line with current practice disease management protocols
* Admitting or discharging patients to and from the caseload andreferring to other care providers as appropriate
* Recording clear and contemporaneous consultation notes to agreedstandards
* Compiling and issuing computer-generated acute and repeatprescriptions (avoiding hand-written prescriptions whenever possible)
* Prescribing in accordance with the practice prescribing formulary(or generically) whenever this is clinically appropriate
* Responsible for medication reviews for repeat meds of patient seen or referred.
* Responsible for medication reconciliation of discharges and any other actions that arise from the discharge.
* Follow up on tasks directed to you.
* In general, the post-holder will be expected to undertake all thenormal duties and responsibilities associated with a GP working withinprimary care.
* Reporting of any significant events and completing necessaryforms.

Other responsibilities within the organisation:

* Awareness of and compliance with all relevant practicepolicies/guidelines, e.g. prescribing, confidentiality, data protection,health and safety
* Awareness and understand of Care Navigation and supporting thepractice with processes.
* Take part in complaints review and responses when required.
* Attend practice meetings and SEA review meetings.
* Understand and compliance with CQC standards.
* Maintain professional registration.
* Daily use of the practice internet system (TeamNet)
* A commitment to life-long learning and audit to ensure evidence-basedbest practice
* Contributing to evaluation/audit and clinical standard settingwithin the organisation
* Contributing to the summarising of patient records and read-codingpatient data
* Attending training and events organised by the practice or otheragencies, where appropriate.

* In the performance of the duties outlined in this job description,the post-holder may have access to confidential information relating topatients and their carers, practice staff and other healthcareworkers. They may also have accessto information relating to the practice as a business organisation.All such information from any source is to be regarded as strictlyconfidential
* Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the practice may only be divulged toauthorised persons in accordance with the practice policies and proceduresrelating to confidentiality and the protection of personal and sensitivedata.

Health & safety:

The post-holderwill implement and lead on a full range of promotion and management of theirown and others health and safety and infection control as defined in thepractice health & safety policy, the practice health & safety manual,and the practice infection control policy and published procedures. This willinclude (but will not be limited to):

* Using personal security systems within the workplace according topractice guidelines
* Awareness of national standards of infection control andcleanliness and regulatory / contractual / professional requirements, andgood practice guidelines
* Providing advice on the correct and safe management of thespecimens process, including collection, labelling, handling, use ofcorrect and clean containers, storage and transport arrangements
* Correct personal use of Personal Protective Equipment (PPE) andensuring correct use of PPE by others, advising on appropriatecircumstances for use by clinicians, staff and patients.
* Management of the full range of infection control procedures inboth routine and extraordinary circumstances (e.g. pandemic or individualinfectious circumstances)
* Hand hygiene standards for self and others
* Managing directly all incidents of accidental exposure
* Management and advice relating to infection control and clinicallybased patient care protocols, and implementation of those protocols acrossthe practice
* Active observation of current working practices across thepractice in relation to infection control, cleanliness and relatedactivities, ensuring that procedures are followed and weaknesses/trainingneeds are identified, escalating issues as appropriate to the responsibleperson
* Identifying the risks involved in work activities and undertakingsuch activities in a way that manages those risks across clinical andpatient process
* Making effective use of training to update knowledge and skills,and initiate and manage the training of others across the full range ofinfection control and patient processes
* Safe management of sharps use, storage and disposal
* Maintenance of own cleanworking environment
* Using appropriate infection control procedures, maintaining workareas in a tidy, clean and sterile and safe way, free from hazards.Initiation of remedial / corrective action where needed or escalation toresponsible management
* Actively identifying, reporting, and correcting health and safetyhazards and infection hazards immediately when recognised
* Keeping own work areas and general/patient areas generally clean,sterile, identifying issues and hazards/risks in relation to other workareas within the business, and assuming responsibility in the maintenanceof general standards of cleanliness across the business in consultation(where appropriate) with responsible managers
* Undertaking periodic infection control training (minimum onceannually)
* Correct waste and instrument management, including handling,segregation, and container use
* Maintenance of sterile environments
* Demonstrate due regard for safeguarding andpromoting the welfare of children.

In addition to maintaining continued educationthrough attendance at any courses and/or study days necessary to ensure thatprofessional development requirements for PREP are met, the post-holder willparticipate in any training programme implemented by the practice as part ofthis employment, with such training to include:

* Participation in an annual individual performance review,including taking responsibility for maintaining a record of own personaland/or professional development
* Taking responsibility for own development, learning andperformance and demonstrating skills and activities to others who areundertaking similar work.

Quality:

The post-holder will strive to maintain qualitywithin the practice, and will:

* Alert other team members to issues of quality and risk
* Assess own performance and take accountability for own actions,either directly or under supervision
* Contribute to the effectiveness of the team by reflecting on ownand team activities and making suggestions on ways to improve and enhancethe teams performance
* Work effectively with individuals in other agencies to meetpatients needs
* Effectively manage own time, workload and resources.

The post-holder should recognize the importance of effectivecommunication within the team and will strive to:

The post-holder will:

* Apply practice policies, standards and guidance
* Discuss with other members of the team how the policies, standardsand guidelines will affect own work
* Participate in audit where appropriate.
