The post-holder will work within a clinical governance framework, committed to continuous improvement and safeguarding standards of care.
Take every opportunity to promote health initiatives and act as a signpost to other relevant services, empowering families.
Maintain a safe environment, implement evidence-based practice, and develop specialized services based on clear evidence of need.
Provide core health visiting services, work in partnership with families, undertake holistic assessments, and review outcomes.
Refer clients to other agencies as necessary, develop protection plans, and undertake clinical tasks like immunizations.
Plan and organize clinics and health promotion programs, manage equipment, and travel to various venues, including homes and community sites.
Contribute to achieving local health targets, manage workload, and coordinate packages of care, collaborating with other professionals.
Undertake the role of Lead Professional when appropriate, as per the Every Child Matters framework.
