West Berkshire Council is seeking a Social Care Practitioner to join our Adult Social Care team, where you will play a vital role in delivering exceptional care and support to our service users. The Role As a Social Care Practitioner, you will be responsible for: Working collaboratively with the Locality Manager to ensure the effective delivery of social care services. Conducting assessments and reviews of individual care needs, ensuring appropriate support is in place. Building positive relationships with service users, their families, and other professionals to promote well-being and independence. Providing advice, guidance, and support to individuals, empowering them to make informed choices. Ensuring compliance with relevant legislation, policies, and procedures. Contributing to the continuous improvement of services through participation in team meetings and development initiatives. The ideal candidate will possess the following skills and attributes: A relevant social care qualification and experience working in a similar role. Sound knowledge of social care legislation, regulations, and best practices. Excellent assessment and care planning skills, with the ability to identify and address individual needs. Strong interpersonal and communication skills, with the ability to build rapport with service users and collaborate with multidisciplinary teams. Empathy, compassion, and a person-centred approach to care. Effective organisational and time management skills, with the ability to prioritise tasks in a fast-paced environment.
