Key Duties
Perform medication reviews and optimisation by conducting comprehensive medication reviews for patients to ensure safe, effective, and appropriate use of medications.Prescribe and re-prescribe medications in accordance with clinical guidelines, patient needs, and organisational long-term conditions by supporting patients with chronic diseases ( diabetes, hypertension etc) by reviewing and optimising treatment plans.Liaising with the MDT, communicate with hospital pharmacists or specialists to discuss complex cases or medications that require specialised successful applicant must be comfortable to mentor trainee support staff and pharmacists.
