Exciting opportunities have arisen across Liverpool. We are seeking a dynamic and compassionate Occupational Therapist to join our multidisciplinary team, with flexible rotational opportunities across both community and bed-based services.
The ICRAS service delivers urgent and short-term reablement and assessment support to individuals either at risk of hospital admission or transitioning out of acute care. Our integrated model promotes admission avoidance, safe discharges, and person-centered care, working in partnership with social care providers, GPs, community clinicians, and other stakeholders.
Key Responsibilities:
1. Deliver high-quality, patient-centred assessments and reablement interventions in community and inpatient settings.
2. Work autonomously and collaboratively within a multidisciplinary team including Advanced Practitioners, Nurses, Pharmacists, Social Workers, Dietitians, and other allied health professionals.
3. Support patients in their own homes or rehab units to regain independence, reduce the risk of hospital admission, and promote safe and timely discharge.
4. Participate in the development, implementation, and evaluation of individualised care plans.
5. Contribute to service development and quality improvement initiatives.
6. Provide supervision and support to junior staff and students as required.
7. Engage with family members, carers, and external agencies to ensure holistic care planning.
Requirements:
8. Qualified and HCPC registered Occupational Therapist or Physiotherapist.
9. Demonstrated experience in a relevant clinical setting (community or acute).
10. Ability to work independently across a range of clinical environments.
11. Full UK driving license and access to a vehicle for work purposes.
12. Excellent communication, decision-making, and interpersonal skills.
13. Flexible and resilient, with the ability to adapt to a changing and dynamic service.
Job Type: Part-time
Pay: Up to £26.00ph
Expected hours: 40 per week
